***** Orthodontic benefits cover adults and children, and begin in year three.
Waiting Periods:
Dental Provisions:
> Employee benefits graduate during the second and third years that they are covered under the plan.
> Child(ren)'s coverage terminates at age 19; or 25 if full-time student.
> Contributory for employees and dependants.
> 30-day initial open enrollment. If coverage is waived, employee must wait for the next open enrollment.
> 30-day annual open enrollment will allow either new hires or current employees who previously
waived coverage, the opportunity to enroll.
